Before My Surgery

In order to make your experience as stress-free as possible, we ask that you complete our pre-registration forms.

If you chose to not complete your pre-registration forms online, you will either receive a copy from your surgeon’s office, or a copy will be mailed to your home.

Once you have completed the forms, you can either mail, FAX, or bring them to the Center.  We request that you arrange to have forms received by the Center at least THREE days prior to your surgery or procedure date.  Forms may be faxed to 949-455-0428.

If you wish to complete your pre-registration by phone, please contact the Center at least THREE days prior to your surgery or procedure date.  Our pre-admission representatives are available to assist you Monday through Friday between the hours of 9:00 AM and 5:00 PM at 949-472-3824.

Prior to your procedure:

You will receive a call from a representative of our Business Office who will review your insurance coverage, and subsequent financial responsibilities to the Surgery Center.  During this conversation, you will be apprised of any co-pays, deductibles, and co-insurance requirements based on your specific insurance plan coverage.  Upon your admission to the Center, we will provide a written explanation of these responsibilities.  Please do not hesitate to ask for clarification of any financial information provided to you.

If you will be having a surgical procedure:

A member of our nursing staff will contact you prior to your surgery date to verify your appointment time and arrival, review your health history, discuss pre-operative preparation instructions and answer any questions you may have.

Please make yourself available by phone the evening before your surgery so that your anesthesiologist may call you if appropriate.
